MOLECULAR EPIDEMIOLOGY OF RISK FACTORS AND TRANSMISSION ROUTE OF HEPATITIS C VIRUS (HCV) AMONG IRAQI PREGNANT WOMEN

Abstract

Objectives: To identify seroprevalence of HCV markers (antibody, RNA, genotypes) and the possible mode of transmission among Iraqi Pregnant women.

Methods: Sera of 3491 pregnant women, tested for anti-HCV positivity using third generation, enzyme immunoassay EIA- 3 and immunoblot assay Lia-Tek III. as screening and confirmatory tests respectively. HCV RNA and genotypes were investigated in 94 mother`s sera using RT PCR and DNA Enzyme immunoassay (DEIA) method at laboratories of Sorin Biomedica – Italy.

Results: Seroprevalence of positive HCV antibody, RNA were detected as (3.21), (62.7%) respectively. Most variables as. History of Blood transfusion, anti-D-immunoglobulin, curettage, abortion, gravidity(=5,) were significantly associated with seropositive HCV markers in which HCV antibodies and RNA were detected as follows (16.3%,80%), (17.49%,77.7%), (6.8%,75%), -(5.35%,83.3), (3.7%,77.7%). respectively. Also we gave evidence that they act as risk factors for acquiring HCV infection by calculating ORs & 95% C.I.; found as follows.; (OR=7.54), (OR=3.32), (OR=3.3), (OR=2.316), (OR=2.14) respectively. Striking blood transfusion found, unconfounding risk factor (adjusted OR=1.938 95%C.I=1.646-2.28) with direct positive linear regression correlation P=0.0056 .No significant association between HCV genotypes and possible mode of HCV infection except HCV-1b significantly associated with history of anti-D immunoglobulin.

Conclusion: This comprehensive epidemiological study gave strong evidence that HCV is nosocomial and blood born infection. Therefore strict blood donor screening and adequate sterilization of medical and surgical instrument are recommended.
